  • 非浸潤性乳管癌発生に関する組織学的研究, HISTOPATHOLOGICAL RESEARCH OF CARCINOGENESIS OF DUCTAL CARCINOMA IN SITU, 小山 徹也; 佐野 孝昭, OYAMA Tetsunari; SANO Takaaki, 日本学術振興会, Japan Society for the Promotion of Science, 科学研究費助成事業, Grants-in-Aid for Scientific Research, 基盤研究(C), Grant-in-Aid for Scientific Research (C), 群馬大学, GUNMA UNIVERSITY, 2002年, 2003年, 本研究では、乳腺の切除材料やcore needle biopsy材料を利用して、早期乳癌発生を検索する目的で、前癌病変を含む乳腺病変のホルモンレセプターとその関連因子、細胞周期関連因子発現検討をおこなった。特に14-3-3σ蛋白やEfp(estrogen responsive RING-finger protein)、p63に注目して検討した。 症例:低悪性度の非浸潤性乳管癌(DCIS)を主体に、特殊型を含めた浸潤癌や良性のcounterpartとして過形成病変の検討も行った。また各種keratin、ER、p63、p53などと対比検討した。また異型嚢状小葉(atypical cystic lobules;別名異型を伴う円柱状過形成CCHA)も検討した。 結果:p63、14-3-3σともに正常小葉の筋上皮細胞に存在した。14-3-3σ発現は、癌では非浸潤癌、浸潤癌と減少した。癌病巣において14-3-3σとERの発現が逆相関する傾向にあった。特殊型ではp63は腺嚢胞癌、14-3-3σは粘液癌と扁平上皮癌で陽性となった。乳腺のpremature cell(stem cell)の分化・増殖へのp63および14-3-3σの関与が示唆された。 1)通常型乳管過形成、非浸潤性乳管癌、浸潤性乳管癌の3病変について ほとんど(81%)の通常型乳管過形成では14-3-3σ強陽性であり、筋上皮細胞に濃く染まったが、17例(65%)では、CK14陽性・actin陰性細胞に陽性所見が見られた。非浸潤性乳管癌では34例中22例(65%)に14-3-3σ強陽性であり、細胞質にびまん性の染色性を示した。1例CK14陽性症例があり、この症例では同じ細胞に14-3-3σ陽性であった。浸潤性乳管癌では71例中16例(23%)で14-3-3σ陽性であり、そのうち半数は強陽性であった。全ての通常型乳管過形成の症例でERα陽性であり、非浸潤性乳管癌(82%)、浸潤性乳管癌(65%)と減少した。p53の陽性率は、それぞれ0%、18%、27%と増加し、ERαと逆相関した。Efpの染色の濃さは様々であったが、いずれの病変でも半数以上の症例で陽性であり、そのうち半数は強陽性であった。 2)異型を伴う円柱上皮過形成(CCHA, columnar cell hyperplasia with atypia)について CCHAにおいて、14-3-3σはほとんどの症例で周囲の筋上皮細胞に限局しており、3例(21%)で乳管上皮細胞にも発現が見られるものがあった。すべてのCCHAはERα陽性であり、p53陽性例は1例も見られなかった。Efpは約半数の症例に陽性だった。 考察:免疫染色結果より、14-3-3σ発現の消失が通常型乳管過形成の8%、非浸潤性乳管癌の35%、浸潤性乳管癌の77%で見られることがわかった。14-3-3σは癌抑制遺伝子であることを裏付けた。14-3-3σ蛋白の減少の機序としては、今回3病変間でEfpの陽性率に差は見られず、乳癌において発現の減少は、メチル化による発現抑制の影響の方が大きいのではないかと考えた。蛋白発現の減少割合は報告によるメチル化の割合よりも低いのは、メチル化が部分的なものであると考えられる。CCHAにおける14-3-3σ発現は通常型乳管過形成より非浸潤性乳管癌に近い染色態度であり、乳癌発癌過程においてCCHAは非浸潤性乳管癌の前がん病変であると考えられた。, It becomes more important to clarify the molecular pathological mechanism of early carcinogenesis and identify the morphological change of breast cancer according to the increase of occurrence of breast cancer. We have already proposed "atypical cystic lobules (ACL)", which is also called as columnar cell hyperplasia with atypia (CCHA). It is one type of early change of low grade ductal carcinoma in situ (DCIS). In this study, we carried out immunohistochemical analysis of breast carcinogens using several antibodies for related proteins, including hormone receptor (Estrogen receptor), hormone related protein (Estrogen-responsive RING finger protein) and 14-3-3 sigma (σ) protein. We have studied the expression of these proteins in usual ductal hyperplasia (UDH), ductal carcinoma in situ (DCIS) and invasive ductal carcinoma (IDC). Immunostaining for estrogen receptor alpha (ERα), p53 and (Efp) was also carried out. Immunohistochemically, expression of 14-3-3σ was seen in 92% UDH lesions and gradually decreased from 65% in DCIS to 23% in IDC. The expression of ERα decreased gradually from UDH to DCIS to IDC, while p53 showed an inverse staining pattern to that of ERα. The expression of Efp showed no significant difference among the three breast lesions. Hence, the present immunohistochemical study confirmed 14-3-3σ as a tumor suppressor in breast carcinogenesis. A similar immunohistochemical analysis was then carried out on columnar cell hyperplasia with atypia (CCHA), in which the expression pattern of tumor suppressor 14-3-3σ, ERα and p53 suggested that the possibility may exist that CCHA is a pre-cancerous lesions., 14570134

  • ヒト正常ならびに腫瘍組織における14-3-3 sigma蛋白の分布解明からその機能解明へ, Study of 14-3-3 sigma protein in normal and neoplastic tissues., 中島 孝; 前田 雅弘; 佐野 孝昭; 小山 徹也; 前田 雅昭, NAKAJIMA Takashi; MAEDA Masahiro; SANO Takaaki; OYAMA Tetsunari, 日本学術振興会, Japan Society for the Promotion of Science, 科学研究費助成事業, Grants-in-Aid for Scientific Research, 基盤研究(B), Grant-in-Aid for Scientific Research (B), 群馬大学, Gunma University, 2002年, 2003年, 14-3-3σ蛋白の分布を検討するために、14-3-3σ遺伝子のC末端配列に基づいたペプチドを合成し、これをウサギに免疫し、14-3-3σ蛋白特異抗体を作製した。正常ヒト組織における14-3-3σ蛋白の発現を免疫組織学的に検討した結果、14-3-3σ蛋白は皮膚,食道,子宮頚部等の扁平上皮に強く発現していること、さらに、気管支上皮,乳腺、消化管上皮等にも広く発現していることを明らかにし、この蛋白が上皮細胞に特異的に発現していることを確認した。また、非小細胞性肺癌においては、腺癌よりは扁平上皮癌に強く14-3-3σ蛋白が発現していることを明らかにした。さらに、肺の大細胞性神経内分泌癌で14-3-3σ蛋白発現がほとんど見られず、この癌は小細胞癌に類似していることが明らかとなった。乳癌では、これまでに報告されたように浸潤性乳管癌の大部分で14-3-3σ蛋白の発現が低下していることを確認したが、特殊型とされる扁平上皮癌や粘液癌では発現が保たれていることを見出した。乳癌や小細胞癌では、14-3-3σ蛋白の発現低下はその遺伝子プロモーター領域のメチル化によって引き起こされることが既に報告されている。しかし、非小細胞性肺癌などでは14-3-3σ蛋白発現が保たれていることが多く、遺伝子プロモーター領域のメチル化が見られないことも既に報告された。そこで、大腸癌における14-3-3σ蛋白遺伝子プロモーター領域のメチル化が見られないことを培養株ならびに手術例を用いて明らかにし、大腸癌における14-3-3σ蛋白発現低下は別の機構によることを明らかにし、これまでの報告とは異なる発現低下機構の存在が示唆された。, In normal human tissues, 14-3-3 σ protein was exclusively present in various epithelial cells, of which squamous epithelia at various sites showed the strongest immunoreactivity and basal cells of respiratory tract or myoepithelial cells of various organs followed. In non-small cell carcinoma of the lung, squamous cell carcinoma expressed stronger immunoreactivity for 14-3-3 σ protein than adenocarcinoma. In human colon cancers, immunoreactivity for 14-3-3 σ protein varied from area to area or case to case. However, there was a tendency to be positive for 14-3-3 σ expression at tumor invasion front. Molecular biological study revealed that DNA methylation of the gene did not influence the expression of the 14-3-3 σ gene in colon cancers. In breast carcinogenesis, down-regulation of 14-3-3 σ protein was well correlated with its progression. In ductal carcinoma lesions, even in non-invasive, marked down-regulation of 14-3-3 σ expression was observed.   • 乳癌微小病変、前癌病変におけるエストロジェンレセプター及び関連物質の検討。特にAIB1(amplified in breast cancer-1)のin situ hybridizationに注目して, EXPRESSION OF ESTROGEN RECEPTOR (ER) AND ER-RELATED PROTEIN IN THE PRECURSOR LESION AND ERALY LESION OF BREAST CANCER, 小山 徹也; 佐野 孝昭, OYAMA Tetsunari; SANO Takaaki, 日本学術振興会, Japan Society for the Promotion of Science, 科学研究費助成事業, Grants-in-Aid for Scientific Research, 基盤研究(C), Grant-in-Aid for Scientific Research (C), 群馬大学, GUNMA UNIVERSITY SCHOOL OF MEDICINE, 1999年, 2000年, 乳腺微小病変、前癌病変におけるホルモンレセプターとその関連物質の蛋白発現の検討。 材料は乳腺手術材料のホルマリン固定パラフィン包埋切片を利用した。奨励は群馬大学附属病院及びKoerner F博士との共同により、MGH症例についても検索した。病変はACL(Atypical Cystic Lobule)を主体とし、これと癌特に低悪性度の非浸潤性乳管癌(microrpapillary type)及びBlunt duct adenosisなどの良性病変を比較検索した。ACLは我々が前癌病変として提唱する核異型を有する嚢状病変である。以下の結果を得た。 1-1 Estrogen Receptor(ER)及びProgestrone Receptor(PgR)に加えて、cyclin D1 FAS(fatty acid synthetase)の蛋白発現の検討 ABC法による免疫染色により検索した。使用した抗体はER、PgR、Ki-67、FAS(fatty acid synthase)、cyclinD1、actin、gesolinである。ER/Ki-67、PgR/FASについては2重染色をして、細胞内局在を検討した。ACL細胞は1)ERの高い発現を見る。2)PgRとFASの瀰慢性陽性所見を有する。3)cyclinD1は散在性に陽性である。4)gelsolinは減少ないし消失している。4)Ki-67陽性細胞は癌と比較して低い。以上よりACLの形質は増殖能が低いことを除けば、低悪性度非浸潤癌との知見を得た。 1-2 Osteopontin(OP)mRNA発現の検討 乳腺組織の微小石灰化に関与する物質としてOP発現を免疫染色及びin situ hybridization(ISH)で検索した。OP蛋白は低悪性度(cribriform type)の非浸潤癌で80%、高悪性度(comedo type)の非浸潤癌で50%、ACLで56%の発現があり、主に組織球及び腫瘍周囲の間質に発現をみた。またISHで組織球内にOP mRNAを確認した。OP発現形式から見るとACLの石灰化は低悪性度非浸潤癌と同様である。 2 Co-activator発現の検討 Steroid receptor co-activator(SRC)としてSRC-1, SRC-3(AIB1)の発現を検討した。免疫染色による検討ではSRC-1は良性、ACL、癌の核いずれにも幅広く存在した。SRC-3(AIB1)は比較的限局した悪性の高い浸潤癌に陽性所見が見られた。ERの発現と比較した局在の検討や免疫染色やin situ hybridiationによる検索は継続する予定である。, A.Atypical cystic lobules of the breast, an early stage of low grade ductal carcinoma in situ. The authors describe the characteristics of atypical cystic lobules (ACLs), which represent a step in the formation of low-grade ductal carcinoma in-situ. The authors defined ACLs as a proliferation of luminal cells showing low-grade cytological atypia without architectural atypia. ACLs were compared with conventional hyperplasia, low-grade ductal carcinoma in-situ and lobular neoplasia. 1) In about forty percent of the cases, atypical cystic lobules merged with fully established micropapillary/cribriform ductal carcinoma in-situ. 2) Immunohistochemical staining for hormone receptors, keratin nineteen, and cyclin D1 revealed that atypical cystic lobules demonstrate a consistent immunophenotype, which differs from the pattern shown by normal lobules and benign lesions and matches the one of low-grade ductal carcinoma in-situ. 3) ACLs are sometimes calcified. Osteopontin-positive histiocytes were infiltrated in all the Kossa-positive (type II microcalcification) cribriform and comedo type carcinomas and ACLs. The similarities in the cytological and immunohistochemical features, the close proximity of the two type of proliferation and the similarities with respect to the types of calcification suggest that atypical cystic lobules represent an early stage in the formation of certain types of low-grade ductal carcinoma in-situ. B.Microcalcification of carcinoma in situ and atypical cystic lobules of the breast with infiltration of macrophages with osteopontin. Abstract We studied the pattern of calcification and the expression of osteopontin (OP) protein and mRNA by the noninvasive components i of twenty breast cancers and sixteen atypical cystic lobules (ACLs). Ten breast cancers (Cr type) showed low-grade cribriform carcinoma in-situ containing secretory material ; the remaining ten cancers (Co type) displayed high-grade carcinoma in-situ with central necrosis. Hematoxylin-eosin and Kossa staining revealed calcium hydroxyapatite calcifications in 80% of Cr type carcinomas, 50% of Co type carcinomas and 56% of ACLs. Immunohistochemical staining demonstrated OP protein in intraluminal secretory material, necrotic debris, or stroma and in histiocytes in all the Kossa-positive carcinomas and ACLs. In situ hybridization revealed OP mRNA mainly in the histiocytes and especially in those near the calcifications. The close relation between hydroxyapatite crystals and OP producing histiocytes suggests that these cells change the microenvironment in a way that facilitates the crystallization of calcium. The similarity in the patterns of osteopontin expression by ACLs and Cr type ductal carcinomas in-situ suggests that the same mechanism gives rise to the calcifications seen in both lesions. C.Expression of Steroid receptor co-activator (SRC)-1 and SRC-3 (=amplified in breast cancer, AIB1) in several breast lesions including precursor lesions. We have tried to detect expression of SRC-1 and SRC-3 in various lesions in the breast, especially in in situ carcinoma and atypical cystic lobules (ACLs) and conventional hyperplasia. SRC-1 was demonstrated almost all nuclei, benign and malignant, in the breast in contrast that estrogen receptor was observed in partial segregated nuclei in proliferative conditions.   • ヒト癌におけるP16-COK-Rb細胞回転異常に関する病理学的研究, 中島 孝; 佐野 孝昭; 柏原 賢治; 小山 徹也, 日本学術振興会, 科学研究費助成事業, 基盤研究(B), 群馬大学, 1999年, 2000年, 肺癌では、扁平上皮癌と腺癌におけるRb経路とp53経路の異常を免疫組織学的に検討した結果、扁平上皮癌ではp16の発現低下が著明にみられた。また、p53経路ではp53が約60%に強陽性であり、点突然変異を起こしていると考えられた。P53陽性所見は病期の進行と共に増加していた。また、病期I期の腺癌を用いた同様の検討では、p14ARFの低下が著明で、MDM2発現亢進がみられ、特に、高分化腺癌に顕著であった。このようなp53経路の状態は胎生期の細胞増殖機構に類似しており、早期の高分化腺癌がこのような増殖機構を利用していることは興味深い。また、腺癌でもp53は細胞分化の低下にともなって、異常が多くなることも明らかとなった。P53については、病期I期の非小細胞肺癌において、患者予後に関連するのはp53免疫染色結果より、その遺伝子における点突然変異であることが既に明らかとなっている。口腔における扁平上皮癌の発癌過程をみると、前癌病変から癌病変にかけて、p53,p16やpRb発現増加、p27発現低下が見られるが、疣贅癌は通常の扁平上皮癌とは異っていることが明らかにされた。さらに、扁平上皮癌について、Rb経路とp53経路の異常という観点から検討すると、p14ARFの低下、MDM2発現亢進を伴うものが認められ、肺の高分化腺癌と同様の所見を示すものが認められ興味深い。このようにRb経路とp53経路の異常は病期によってかなり異なることが明らかとなったが、ヒト癌においてはp53の点突然変異が最も頻度の高いp53経路の異常を引き起こす異常であることが再確認できた。その他、食道癌ではp16の発現低下が著明であること、乳癌ではp21がBRCA1メチル化と関連することなどを明らかにしてきた。, 11470049
